Punjab Mukh Mantri Sehat Bima Yojana (AB-PMJAY MMSBY) 2025 – Registration, List of Beneficiaries, Hospitals, Treatment & Packages

1. What the  is Punjab Mukh Mantri Sehat Bima Yojana

  • The scheme is a state‑health‑insurance / cashless treatment cover scheme for Punjab residents, combining the earlier schemes Ayushman Bharat – Sarbat Sehat Bima Yojana (AB‑SSBY) and a state top‑up under Ayushman Bharat PM‑JAY Mukh Mantri Sehat Bima Yojana (AB‑PMJAY MMSBY).
  • In July 2025 the Punjab government announced that all families in Punjab will get cashless treatment cover up to ₹10 lakh per year.
  • The scheme is said to benefit the entire population of the state (~3 crore residents) and the rollout date for the new universal cover is from 2 October 2025.

2. Eligibility & who can register

Eligible persons

Previously, under AB‑PMJAY MMSBY / SSBY the categories were (and many still apply):

  • Families as per SECC 2011 data in Punjab.
  • Smart Ration Card‑holder families.
  • J‑Form holder farmers / small & marginal farmers.
  • Small traders registered with the state Excise & Taxation Department.
  • Accredited journalists (yellow card).
  • Construction workers registered with the Punjab Building & Other Construction Workers Welfare Board.

Universal access from 2025

  • With the July 2025 announcement, the cover is extended to all families/residents of Punjab, regardless of income, age or gender.
  • So even if a family was not previously in the “eligible list”, under the new scheme they may be entitled from 2 October 2025 onwards.

Documents required for registration

Typical documents required include:

  • Aadhaar card.
  • Ration Card / family ID.
  • For specific categories: Construction‑worker card, Small trader registration proof, etc.

3. Registration / How to apply

  • You can register / generate the e‑card (health card) via the official portal of the state health agency (State Health Agency, Punjab) or through common service centres / Sewa Kendras.
  • Steps typically: visit the website → choose eligibility check/registration → enter Aadhaar / family ID / mobile → OTP verification → e‑card generation.
  • Beneficiary list and e‑card status: you can check online via the portal (enter Aadhaar / ration card number) whether your family is included.

4. Coverage, benefits & treatment packages

Sum insured

  • Under the earlier scheme the cover was up to ₹5 lakh per family per year for secondary & tertiary care.
  • From the new announcement: up to ₹10 lakh per family per year for all families in Punjab, starting 2 October 2025.

What is covered

  • Cashless & paperless in empanelled hospitals (public & private) in Punjab.
  • Secondary and tertiary care hospitalisation, surgeries, etc.
  • Pre‑hospitalisation (typically ~3 days) and post‑hospitalisation (~15 days) expenses included.
  • Treatment of newborns, day‑care surgeries, and even pre‑existing diseases may be covered.

Treatment packages

  • The scheme (under its earlier version) covers ~1,579 packages for secondary & tertiary care.
  • For example, the website mentions that under SSBY/AB‑PMJAY MMSBY there are packages across cardiology, neurosurgery, orthopaedics, cancer treatments, etc.

5. Empanelled Hospitals & Treatment network

  • The scheme works through “empanelled hospitals” (both government & private) in Punjab and in some cases outside the state (depending on empanelment).
  • As per July 2025 announcement: more than 550 private hospitals enrolled so far; target to increase to ~1,000.
  • To check hospital list: go to the SHA Punjab website → “Hospitals” tab → select district, speciality, etc.

6. Checking Beneficiary List / Status

  • You can check if your family is eligible or already enrolled via the portal: use Aadhaar number, ration card number or family ID.
  • You can also check the status of your e‑card: “Active/Inactive” status via portal.

7. Key dates & rollout

  • Scheme originally launched on 20 August 2019 (for AB‑SSBY / MMSBY portion) in Punjab.
  • In July 2025 the cabinet approved the expansion to ₹10 lakh universal cover from 2 October 2025.

8. Important things to keep in mind & tips

  • Even if you don’t have a card yet, from 2 October 2025 you may become eligible under the universal regime — so it’s wise to check your status ahead of any hospitalization.
  • When going for treatment, carry your e‑card (or proof of registration), Aadhaar, ration card / family ID and hospital should be empanelled under the scheme.
  • Check the list of empanelled hospitals in your district/speciality ahead of time — ensure that the hospital accepts the scheme.
  • Keep a copy of pre‑hospitalisation & post‑hospitalisation details if you anticipate lengthy care.
  • If your name is not appearing in the list, contact the nearest Seva Kendra / CSC / hospital’s scheme help desk, or the SHAPunjab helpline.
  • Retain all documentation / cards that you may have received under earlier scheme versions (₹5 lakh cover) — they may be useful for continuity.
  • If treatment is outside Punjab (but hospital is empanelled for PM‑JAY or associated network) check ahead if the scheme covers it as per 2025 rollout.
